CHILD REFERRAL

This form can be used by Schools to refer a pupil to the section, or by a parent to request their son or daughter to be included on the names list. We must stress that completion of the form may not necessary mean an invitation to one of our activity camps will be made. As you can understand interest can be high for these camps and we must consider each referral on its merits. However the form will be kept on record and inclusion on a future event may be possible.

A member of the Special Needs team will contact the parent of the child directly with any offer of a place on the activity, with full details. Further medical details may be requested by the nursing staff directly with the School / Parent to enable proper care on activities this will only take place with the permission of the legal guardian.
